Report a Lost, Stolen, or Damaged
Library Card or a Change of Name

Use this form to report a lost, stolen, or worn library card and request a new card, or cancel your library card, or cancel that of your child. You may also update your library card online.

As a security measure to ensure your privacy, you will be required to show proof of identity and your mailing address when you pick up your new card.

Your new library card should be ready for pickup approximately 4 hours after sending your registration.
