Minor Library Card Registration

 

Thank you for your interest in getting a library card. Just fill out the form below, and we'll get you registered.

 

As a security measure to ensure your privacy, you will be required to show proof of identity and your mailing address when you pick up your child's card.

 

Your library card should be ready for pickup approximately 4 hours after sending your registration.

I give my minor child permission to check out audiovisual equipment valued at more than $100.00. Minors under the age of 18 cannot check out presentation projectors without a parent present. (Some audiovisual equipment is valued at up to $3,500.00)

In return for the right and privilege to use the Campbell County Public Library, I agree to return books and other items when due.

I hereby assume all financial responsibility including but not limited to damage, repair, or replacement costs for print or non-print materials loaned to me.

I further agree that I will be responsible for all materials or equipment checked out on my card up to the date the Campbell County Public Library System receives notification in writing that the card is lost or stolen.

I understand that the Adult Collection is available to children. I accept full responsibility for all selection of materials/equipment and any loss or damage incurred through my child's use of the library.
